New students need the following:
- Consent to Treatment Form
- Physical (within last 12 months)
- Internet Acceptable Use Policy
- Student Permissions Form
- Home & School Volunteer form
- Reference form (Kinder-8th grade – need one form from previous teacher if possible)
- Copy of Birth Certificate
- Copy of most current Immunization records
- Recent Achievement Test results
- Report card from previous year (indicating promotion)
Returning students need the following:
- Consent to Treatment
- Over-the-Counter Medications Form
- New physical for students entering 5th grade
- Internet Acceptable Use Policy
- Student Permissions Form
- Home & School Volunteer Form
